The Institute's Stanford-based program "Optimizing Life" provides lay-led self-management workshops for patients and caregivers to enable them to enjoy physical
and emotional health when living with long-term health conditions.
In this program, participants learn skills and gain the confidence and desire to take charge of their own health and to regain control of their life. The program also provides effective collaboration between patients and healthcare professionals to enhance adherence to therapy and optimize benefits
"Optimizing Life" is the Institute’s Stanford-based Chronic Disease Self-Management Program. The Institute provides four levels of program support for Optimizing Life:
Health Coaching is not a program but is a system of evidence-based principles and techniques that have been built into a
structure that guides health professionals
in how to facilitate health behavior change
in their patients or clients, for better health outcomes.
The processes actively identify and address behavioral, emotional, situational and cognitive barriers to change and build patient skills in decision making, problem solving and planning and skills for promoting and supporting health behaviour change.
